PRN Physical Therapist - ProHealth Home Health and Hospice (Troup County, GA.)
A Registered Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and the plan of care under the direction and supervision of the Director of Clinical Services/Nursing Supervisor.
Qualifications:
- Graduate from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
- Currently licensed in the state(s) in which practicing.
- Two (2) years’ experience, preferred.
- Acceptance of philosophy and goals of this Agency.
- Ability to exercise initiative and independent judgment.
Responsibilities:
- Understand and adhere to established policies and procedures.
- Provide physician-prescribed physical therapy.
- Improve or minimize residual physical disabilities of the patient.
- Return the individual to an optimum and productive level within the patient’s capabilities.
- Participate with all other home care personnel in patient care planning.
- Direct and supervise personnel as required.
- Take initial history and make initial evaluation.
- Perform all skilled procedures as ordered by physician.
- Consult with physicians regarding changes in treatment.
- Write reports to physicians regarding patient’s progress.
- Instruct patients and family/significant others in home programs and activities of daily living.
- Participate in in-service programs and present in-service programs as assigned.
- Participate in Quality Assessment and Performance Improvement activities as assigned.
- Attend all scheduled patient care conferences.
- Prepare clinical and progress notes.
- Assist physicians with evaluating level of function.
- Help develop and revise the plan of care as necessary.
- Consult with family and Agency personnel.
- Complete and submit OASIS assessments, reassessments, transfers, resumption of care, discharges, and significant changes in condition in accordance with Agency-defined timeframes.
- Appropriately utilize ICD-10 codes.
Working Environment:
Works indoors in the Agency office and patient homes, and travels to/from patient homes.
Job Relationship:
- Supervised by: Director of Clinical Services/Nursing Supervisor
- Workers Supervised: Physical Therapy Assistant and Home Health Aide as assigned

Lifting Requirements:
Ability to perform tasks such as physical activity, extended standing, heavy lifting, and extensive bending, lifting, and standing on a regular basis.
