Job summary

Continuum Therapy Partners in Minnesota seeks a licensed Physical Therapist to deliver patient-centered care across our multi-site network, with a PRN schedule at Aitkin Health Services in Aitkin, MN.

You will assess patients, develop and implement individualized plans, document outcomes, and educate patients and families on home exercise programs to prevent injuries and improve quality of life.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
