Job summary

Harvest Healthcare Services is seeking a Physical Therapist (PT) to evaluate residents, design individualized treatment plans, and provide physical therapy in a skilled nursing setting at our Alliance location.

The PT will collaborate with an interdisciplinary team to improve mobility, strength, balance, and independence, while ensuring compliance with standards of practice and regulations. Strong communication and ongoing professional development are required.

Qualifications

• Requires a Bachelor of Science in Physical Therapy and NPTE, with current PT license in the practicing state.
• Prior experience in skilled nursing, long-term care, rehabilitation, or geriatric care preferred.
• Excellent communication and interpersonal skills; ability to work with an interdisciplinary team and commit to ongoing professional development.

Responsibilities

• Evaluate residents to determine PT needs and develop treatment plans.
• Conduct initial and ongoing evaluations with goals.
• Provide PT treatments per plan of care and physician orders.
• Monitor resident responses and adjust interventions to meet goals.
• Collaborate with interdisciplinary team to optimize resident function.
• Deliver education to residents, families, and staff on PT procedures and programs.
• Supervise PTAs and aides as required by state guidelines.
• Maintain accurate documentation per regulations and facility procedures.
• Participate in QA programs and compliance with HIPAA and safety standards.
