Physical Therapist (PT) – Long-Term Care

LP Therapy is seeking a licensed Physical Therapist (PT) to provide skilled therapy services in a Long-Term Care setting focused on restoring mobility, strength, and function.

Responsibilities
  • Review physician referrals and medical records
  • Evaluate strength, mobility, and functional ability
  • Develop and modify treatment plans
  • Administer therapeutic exercises and modalities
  • Educate patients and families
  • Complete timely Medicare documentation
  • Collaborate with care teams
  • Supervise PTAs and aides as required
  • Support quality outcomes and productivity standards
Qualifications
  • Bachelor's degree in Physical Therapy
  • Current state PT license
  • LTC experience preferred
  • Strong knowledge of Medicare guidelines
  • Effective communication and teamwork skills
Work Environment
