Flagship Therapy seeks a PRN Physical Therapist to join Draper Rehabilitation and Care Center in Draper, UT. The role focuses on providing in-house rehab services for geriatric patients within a post-acute setting, delivering evidence-based treatments and collaborative care with the therapy team.

The ideal candidate holds a PT license and is open to all experience levels, with the flexibility to work up to 15 hours per week as needed.

Qualifications

• PT license is required.
• Open to PTs at all experience levels.
• PRN position with up to 15 hours per week may be accommodated.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop plans of care.
• Use standardized assessments and evidence-based practices to guide interventions.
• Provide rehabilitative, skilled and medically necessary treatment per regulatory standards.
• Administer procedures as part of rehab plans (ADL retraining, therapeutic activities, exercises, modalities).
• Monitor patient progress and adjust treatments in the medical record.
• Evaluate outcomes and refer as needed based on results.
• Offer counseling to patients, families, and other providers about physical disorders.
• Communicate progress and plans with supervisor and health team members.
• Prepare comprehensive discharge summaries in line with clinical requirements.
