PRN Physical Therapist - Great Plains Nursing & Rehab

Join our team as a PRN Physical Therapist at Creative Solutions In HealthCare.

We are seeking a dedicated PT to provide person-centered care to our residents, promoting positive outcomes.

Responsibilities
  • Consult with patients about their physical conditions.
  • Assess and interpret patient evaluations and test results.
  • Develop and implement treatment plans using various techniques.
  • Create personalized rehabilitation programs.
  • Administer prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
  • Ensure treatments align with the established care plan.
  • Monitor patient responses and adjust treatments accordingly in collaboration with the interdisciplinary team.
  • Provide education and support to patients, families, and caregivers.
  • Collaborate with team members to ensure quality care.
  • Complete thorough and timely documentation in compliance with regulations.
  • Attend meetings as required by the Director of Rehab.
Skills and Requirements

  • Familiarity with Net Health Electronic Documentation System is a plus.
