Job summary

Lebanon Health and Rehabilitation Center is seeking a Physical Therapist (PT) for our facility. The PT will assess referred patients, establish and administer treatment programs, document patient status, and educate patients and families in physical therapy methods.

Candidates must be a graduate of an accredited Physical Therapy program with a valid license, and they may supervise PT assistants while collaborating with the rehabilitation team.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not required.
• Well versed with documentation writing.

Responsibilities

• Assess referred patients requiring rehabilitation and develop treatment plans.
• Deliver direct patient care and monitor progress.
• Educate patients and families on physical therapy methods.
• Supervise Physical Therapy Assistants and coordinate with the Rehabilitation Director.
