Continuum Therapy Partners is seeking a PRN Physical Therapist to join our team in Milwaukee, WI, at Sunrise Health Services. You’ll have the freedom to build a schedule around your life while contributing to high-quality patient care.

Requirements include a Doctor of Physical Therapy (DPT), an unencumbered Wisconsin PT license (NPTE required), and preferred 1 year of PT experience. As a PRN role, you still collaborate with a team of clinicians who value progress and outcomes.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy license; NPTE required.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
