Job summary

Packers Staffing in Oak Creek, WI is seeking a Physical Therapist (PT) to join a partnered facility on a Per Diem/PRN basis. You will plan, organize, develop and direct Physical Therapy services in line with standards and regulations, under the direction of the Administrator.

This role supports residents, conducts assessments, develops treatment plans, documents in NetHealth EMR, and collaborates with the interdisciplinary team.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in Wisconsin.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Evaluate residents to obtain data for treatment planning and implementation; develop short and long-term goals.
• Develop treatment plans and communicate evaluation findings to residents and interdisciplinary team.
