Packers Staffing is hiring a Physical Therapist PT to join our partnered facility in Burlington, WI. You will provide physical therapy services to residents in accordance with established standards, develop treatment plans, and work with an interdisciplinary team to ensure quality resident care.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in Wisconsin.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Develop treatment plans with short and long-term goals and implement them.
• Conduct screenings to determine need for intervention/treatment and communicate findings to residents and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR system.

Education
