Job summary

Yanceyville Rehabilitation And Healthcare Center in Yanceyville, NC is seeking a PRN Physical Therapist to join our long-term care team. You will perform comprehensive PT evaluations, develop treatment plans, and document progress for residents in accordance with MD orders.

Qualifications

• Graduate degree in Physical Therapy (B.S., Doctorate, MA or MS) recognized by the APTA.
• Holds current license/registration in the state where applicable.

• Document findings on the evaluation form per guidelines.
• Create a comprehensive treatment plan with goals, frequency, duration, and modalities.
• Obtain MD orders for evaluations, treatments, re-certifications and discharges.
