As part of a patient‑centered, collaborative environment, you will assess mobility, provide therapy, educate families, and maintain thorough documentation. This role emphasizes building relationships with residents and enhancing mobility and independence.

Qualifications

• Degree in Physical Therapy from an accredited institution.
• Valid state licensure as a Physical Therapist, or license eligible.
• Strong clinical assessment and diagnostic skills. Proficient in developing and implementing effective treatment plans.
• Excellent communication and interpersonal skills with a patient-centered approach.

Responsibilities

• Assess patients' physical conditions including mobility, strength, and flexibility.
• Provide therapeutic exercises, manual therapy, and other treatments; educate patients and families on home routines.
• Maintain accurate documentation of evaluations, progress, and treatment plans.
• Collaborate with healthcare providers and participate in multidisciplinary team meetings.
