Qualifications

• Bachelor of Science (B.S.) or greater in Physical Therapy.
• 3 years IP: experience preferred; EMR entry experience preferred.
• Current American Heart Association or American Red Cross BLS preferred at time of hire; required within 3 months; PT license to practice in work state.

Responsibilities

• Evaluate a wide variety of patient types and identify physical dysfunction and functional capabilities to facilitate treatment.
• Establish and implement patient-focused treatment plans with regular follow-up to maximize functional improvement.
• Document thoroughly and timely across initial evaluation, progress notes, discharge summaries and progress reports.
• Provide patient education to facilitate rehabilitation; utilize interpreters or support as needed.
• Assist with program development, department marketing strategies, and community education.
