Job summary

HealthPRO-Heritage in Millbrook, NY is seeking a PRN Physical Therapist Assistant to join our team. The PTA will provide direct resident care under the supervision of the Physical Therapist and contribute to treatment planning and documentation.

Responsibilities include administering treatments, assisting with evaluations, maintaining records, and collaborating with families and healthcare providers to support resident goals and outcomes.

Qualifications

• Graduate of an approved Physical Therapy Assistant program.
• Current license or eligible for license in NY.
• Experience with the same age/resident population preferred.
• Excellent oral and written communication and interpersonal skills.
• Ability to follow the Physical Therapist's directions and facility procedures.
• Ability to perform required activities safely (lifting, standing, pushing up to 50 lbs).

Responsibilities

• Provide physical therapy treatments to residents per the therapist's plan.
• Assist the PT with tests, evaluations, and complex treatments.
• Observe, record and report patient responses to the therapist.
• Maintain daily treatment records and progress notes.
• Assist with admission screening and with family/physician communications.
• Support rehab team conferences and comply with policies and safety programs.
