Job summary

Heritage Crossings in Geneva is seeking a PRN Physical Therapist Assistant to join our senior living therapy team. As a PTA, you’ll implement rehab programs on a part-time basis, helping residents stay active, independent, and engaged in the life they love.

Collaborate with the supervising Physical Therapist to tailor treatment, monitor progress, document outcomes, and support mobility and safety in daily activities.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Active Nebraska Physical Therapist Assistant license.
• Strong clinical, communication, and organizational skills.
• Ability to build positive relationships with residents, families, and team members.
• Desire to provide person-centered care with professionalism and compassion.

Responsibilities

• Implement individualized treatment programs developed by the supervising Physical Therapist.
• Support residents in reaching their rehabilitation goals through therapeutic exercises and functional mobility training.
• Monitor resident performance and communicate progress, concerns, and recommendations to the therapy team.
• Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines.
• Collaborate with nursing staff and interdisciplinary team members to promote successful outcomes.

Heritage Crossings is looking for a dedicated Physical Therapist Assistant (PTA) to provide high-quality rehabilitation services on a PRN basis. Working alongside an experienced therapy team, you'll help residents improve mobility, restore function, and achieve meaningful outcomes in a supportive senior living environment.
