Job summary

Heritage Crossings is seeking a dedicated Physical Therapist Assistant (PTA) to provide high-quality rehabilitation services on a PRN basis. You will work alongside an experienced therapy team to help residents improve mobility and achieve meaningful outcomes.

The role requires a Nebraska PTA license and a passion for person-centered care. We offer flexible scheduling, opportunities for professional development, and a supportive team environment to help our residents thrive.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Active Nebraska Physical Therapist Assistant license.
• Desire to provide person-centered care with professionalism and compassion.

Responsibilities

• Implement individualized treatment programs developed by the supervising Physical Therapist.
• Support residents in reaching their rehabilitation goals through therapeutic exercises.
• Monitor resident performance and communicate progress to the therapy team.
• Maintain accurate documentation in accordance with guidelines.
• Collaborate with nursing staff to promote successful outcomes.
• Encourage resident participation while fostering confidence and independence.

• Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines.
• Collaborate with nursing staff and interdisciplinary team members to promote successful outcomes.
• Encourage resident participation while fostering confidence, safety, and independence.

What We're Looking For

• Graduate of an accredited Physical Therapist Assistant program.
• Active Nebraska Physical Therapist Assistant license.
• Strong clinical, communication, and organizational skills.
• Ability to build positive relationships with residents, families, and team members.
• Desire to provide person-centered care with professionalism and compassion.
