American Senior Communities seeks a PRN Physical Therapist Assistant to cover up to three days per week at Cypress Grove Rehab Center in Newburgh, IN. The PTA will work under the supervision of a licensed physical therapist and provide skilled therapy across ASC settings, including home health, SNF and outpatient services.

The role emphasizes team collaboration, discharge planning support, and adherence to state regulations while serving senior residents in local ASC facilities.

• Ability to provide services across ASC Therapies & Wellness settings including home health, SNF, outpatient therapies.
• Monitors and documents patient progress.
• Collaborates on discharge plans and case management.
• Maintains positive interactions with patients and center staff.
• Adheres to applicable state regulations concerning occupational therapy.
