Job summary

HealthPro Heritage in New Haven, CT is seeking a PRN Physical Therapist Assistant to support our therapy teams across facilities. You will assist in implementing treatment plans, provide direct patient care, and document progress under supervision.

Qualifications

• Associate degree in Physical Therapist Assisting from an accredited program.
• Valid state licensure as a Physical Therapist Assistant, or license eligible.
• Strong assessment and treatment planning abilities with clear communication.

Responsibilities

• Assist in implementing treatment plans designed by Physical Therapists and provide direct patient care.
• Monitor and document patient progress, and educate patients and families on protocols.
• Accurately document treatments, progress and responses in the EHR.
• Collaborate with therapists and staff to ensure coordinated care.
