Job summary

Continuum Therapy Partners is seeking a Physical Therapist Assistant to join our team on a PRN daytime schedule at Elroy Health Services in Elroy, WI. The PTA will implement treatment plans under supervision, document patient progress, and educate patients on home exercises and assistive devices.

Ideal candidates hold an accredited PTA degree, current state license, and may have 1 year of PTA experience. This role supports our therapist-led approach across a growing national network.

Qualifications

• Current, unencumbered state PTA license.
• Associate degree in PTA program is required.
• 1 year of previous PTA experience preferred.

Responsibilities

• Implements individualized treatment plans under supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.
