Job summary

Rehab Solutions in Ada, Oklahoma is seeking a PRN Physical Therapist Assistant to join our Home Health team. You will provide direct patient care, implement treatment plans, and monitor progress in a flexible, patient‑centered environment.

Qualifications

• Associate’s degree in Physical Therapist Assistant from an accredited program.
• Valid state licensure as a Physical Therapist Assistant, or license eligible.
• Strong assessment, diagnostic and therapeutic abilities with excellent communication and interpersonal skills.
• Proficient in using diagnostic tools, therapy equipment, relevant software, and EHR systems; ability to collaborate in a multidisciplinary team.

Responsibilities

• Assist in implementing treatment plans designed by Physical Therapists; provide direct patient care including exercises, manual therapy, and modalities.
• Monitor and document patient progress; report observations to the Physical Therapist; educate patients and families on protocols and self-care.
• Accurately document patient treatments and progress in compliance with facility policies.
• Collaborate with Physical Therapists and other health professionals to ensure coordinated care.
