Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a SNF setting. You will evaluate and treat patients according to an individualized plan of care, while delivering excellent patient and family service.

The role includes supervising Physical Therapist Assistants and ensuring thorough documentation of evaluations and progress. Collaborating with facility staff, you will set measurable, functional goals and adjust plans as patients progress, with ongoing education

Responsabilidades

• Evaluate and treat patients following an individualized plan of care.
• Collaborate with facility staff to ensure excellent care for patients.
• Re-evaluate patient progress and adjust plans of care.
• Complete timely and accurate documentation including evaluations, re-evaluations discharges and daily progress notes.
• Educate patients on their injury, progress and expected outcomes.

Enhance Rehabilitation is currently seeking qualified Physical Therapists candidates to join our dynamic team. As an Enhance Rehabilitation Physical Therapist, you will be a part of growing team providing the highest quality care for patients in SNF setting. In this role, you will evaluate and treat patients following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
