Job summary

Kids for the Future in Mishawaka, IN seeks a Pharmacist PRN to support patient care and medication safety on an as-needed basis. You dispense, counsel, verify prescriptions, and participate in DUR, pharmacotherapy planning, and immunization.

You will work with clinicians, maintain accurate records in EHR, perform sterile compounding, ensure compliance with pharmacy laws, and contribute to quality improvement and inventory management.

Qualifications

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
  • Active and unrestricted Pharmacist license in the state of practice.
  • At least one year of experience in a clinical or retail pharmacy setting.

Responsibilities

  • Dispenses medications accurately and efficiently, ensuring proper labeling and packaging.
  • Counsels patients on medication use, potential side effects, and adherence, promoting understanding and compliance.
  • Verifies prescriptions for accuracy, completeness, and appropriateness, identifying and resolving potential issues.
  • Conducts Drug Utilization Reviews (DUR) to evaluate medication appropriateness, detect potential adverse drug reactions, and optimize therapy.
  • Applies Pharmacotherapy knowledge to assess patient needs and recommend evidence-based regimens.
  • Ensures adherence to all federal, state, and local Pharmacy Law and regulations.
  • Provides accurate and timely Drug Information to healthcare professionals and patients.
  • Utilizes EHR systems effectively for documentation, order entry, and patient information retrieval.
  • Prepares Sterile Compounding products safely and precisely following established protocols.
  • Administers Immunization to patients as authorized and trained.
  • Collaborates with physicians, nurses, and other healthcare providers to optimize patient care plans.
  • Maintains inventory control and manages medication stock to prevent shortages or waste.
  • Participates in quality improvement initiatives and contributes to pharmacy policies and procedures.
