About The Job

Job Title/Position: Registered Nurse Case Manager
Reports To: DPCS
Position Objective
The registered nurse plans, organizes, and directs hospice care, with experience in nursing, emphasizing community health education/experience. The professional nurse builds from community resources to plan and direct services that meet the needs of individuals and families within their homes and communities.
Requirements
Travel – This is a traveling position. Must maintain a valid driver’s license, active auto insurance meeting state minimum requirements, and an operational vehicle.
Compensation: $40 - $58 an hour
