About The Job

Job Title/Position: Registered Nurse Case Manager

Reports To: DPCS

Position Objective

The registered nurse plans, organizes, and directs hospice care, with experience in nursing, emphasizing community health education/experience. The professional nurse builds from community resources to plan and direct services that meet the needs of individuals and families within their homes and communities.

Requirements

• Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.
• Current licensure in the state and BLS Hands-On CPR certification.
• Bachelor’s degree, with one (1) year of hospice care experience preferred.
• Management experience is not required but responsible for supervising hospice aides.
• Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills; nursing skills per competency checklist.
• Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment.
• Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills.
• Must be a licensed driver with an insured automobile in good working condition.
• Holistic, complementary, and/or alternative modality training and experience preferred.

Travel – This is a traveling position. Must maintain a valid driver’s license, active auto insurance meeting state minimum requirements, and an operational vehicle.

Compensation: $40 - $58 an hour
