Job summary

Four Seasons seeks a PRN Palliative Care Provider to deliver compassionate symptom management for patients with serious illnesses in homes, facilities, clinics, and hospitals within Western North Carolina. You will join an interdisciplinary team under the Regional Director of Palliative Care and provide education, care coordination, and documentation.

The role requires Masters/Doctorate level training, active NC NP/PA licensure, and DEA registration.

Qualifications

  • Masters or Doctorate level in a related field.
  • 2 years of clinical experience preferred, with palliative care experience preferred.
  • NC NP or PA license in good standing, DEA registration, and driving privilege.

Responsibilities

  • Provide palliative care consultations and follow-up with patients and families.
  • Deliver care in patients' homes, ALF/SNF, outpatient clinics and hospitals.
  • Coordinate care with an interdisciplinary team and referrals.
  • Maintain accurate EMR records and timely documentation.

Summary

Provide palliative care consultation and treatment to patients with serious and life-limiting illnesses in their homes, skilled nursing facilities, assisted living facilities, outpatient clinic, and hospital.

Essential Duties & Responsibilities
  • Provide palliative care consultation and follow-up to patients and their involved persons. Consultations/follow up will include: education, therapeutic communication, and management of symptoms related to life-limiting conditions. Care will be performed in coordination with an interdisciplinary team, which will include: a collaborating physician, registered nurse, and social worker. Chaplain and musical therapist support is also available
  • Provide Palliative Care in patient’s homes, ALF/SNF, outpatient clinic, and hospital locations and submit mileage reimbursement monthly.
  • Participate in ongoing learning. This will include, but is not limited to: learning symptom management of complex illnesses, communication skills, and prognostication
  • Coordinate care and communicate with referral sources.
  • Maintain professional appearance and presentation.
  • Work to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies
  • Actively participate in Interdisciplinary Team meetings for development and evaluation of the clients plan of care
  • Create and maintain accurate patient records within the electronic medical record and complete per policy timeframe
  • Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
  • Serve as clinical resource for staff of Four Seasons, staff at facilities served, and providers in the community
  • Maintain current clinical education and appropriate licensure
  • Perform other duties as requested or assigned

Minimum Qualifications
Education
  • Masters or Doctorate Level
Experience
  • 2 years of clinical experience as a medical provider is preferred
  • Palliative care experience is preferred
Licensures & Certifications
  • Current NP or PA Board Approved Certification
  • Current NC State License in good standing
  • Current DEA Registration in good standing
  • Meet and maintain eligibility requirements to be appointed medical staff and qualify for privileges at partner facilities
  • Valid driver’s license and proof of automobile insurance.

  • Occasional night and weekend phone call coverage, shared among a large clinical team of providers
  • Willingness to drive a variety of terrain and miles between patient locations
