Job summary

Highland Care Center in Salt Lake City, Utah, seeks a PRN Outpatient Physical Therapist or Physical Therapist Assistant to join our dynamic rehab team. You will provide high-quality, patient-centered care in an outpatient setting, including evaluation, treatment, and education.

Work with a multidisciplinary team to develop and monitor individualized plans; perform initial evaluations, reassessments, and progress notes; educate patients and caregivers on exercises, body mechanics, safety;

Qualifications

• Current licensure as a Physical Therapist or Physical Therapist Assistant in Utah.
• Strong communication and interpersonal abilities.
• Ability to work effectively in a fast-paced environment.
• Attention to detail and strong organizational skills.

Responsibilities

• Provide PT and PTA services in outpatient settings, including evaluation, treatment, and education.
• Collaborate with multidisciplinary team to develop and implement treatment plans.
• Conduct initial evaluations, reassessments, and progress assessments to track patient progress.
• Implement and modify treatment plans based on patient response and goals.
• Educate patients and caregivers on exercise programs, body mechanics, and safety.
• Document all patient encounters and plans timely and accurately.
• Participate in quality improvement initiatives to ensure excellent outcomes.
