Beacon Harbor Healthcare in Rockwall, TX is seeking a PRN Physical Therapist to support outpatient ALF programs in Garland and Rowlett. The role involves evaluating patients with physical functioning disorders, creating treatment plans, and delivering skilled interventions in a post-acute setting.

Qualifications

• Active Physical Therapist license is required.
• Open to Physical Therapists at all experience levels; new grads welcome.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care, and develop care plans.
• Deliver rehabilitative and medically necessary treatment interventions.
• Perform procedures as part of the rehabilitation plan (manual techniques, ambulation, therapeutic exercises, modalities, assistive devices, wound care).
• Provide counseling to patients, families, and caregivers regarding physical disorders.
• Generate discharge summaries in compliance with regulatory requirements.
