Job summary

Evolve Therapy Services, LLC in Meyersdale, PA, seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. This PRN role plans, delivers, and directs occupational therapy services to residents in accordance with federal, state, and local standards.

The successful candidate will hold an Associate's degree in Occupational Therapy, have an active OTA license in the applicable states, and engage in ongoing professional development to maintain licensure.

Qualifications

• Active OTA licensure in the state(s) of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screenings of residents to determine interventions per scope of practice.
• Perform treatment plans and follow short- and long-term goals under supervision.
• Interpret and communicate evaluation findings to residents, families, and the team.
• Establish resident therapy schedules in cooperation with team members.
• Document and bill OT services according to plan of care.
• Perform all other duties to effectively treat patients.
• Comply with CMS documentation standards and daily charting.
