Job summary

Renewal Rehab, part of Enhance Therapies, is seeking an Occupational Therapist for PRN renewal assignments in a SNF/Long-Term Care setting in Mundelein, Illinois. The role requires licensure to practice and NBCOT certification where required.

You will evaluate and treat patients with functional and cognitive impairments, develop personalized care plans, collaborate with a multidisciplinary team, and maintain complete documentation to meet regulatory requirements.

Qualifications

• Licensed Occupational Therapist to practice in the state
• OT degree from AOTA-accredited program
• NBCOT-OTR certification if required by the state

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments
• Create and implement personalized care plans
• Collaborate with team members to ensure quality outcomes
• Maintain documentation to meet all regulatory requirements
• Ensure all physician orders are obtained and up to date
