Job summary

Renewal Rehab, part of Enhance Therapies, is seeking an Occupational Therapist for PRN renewal rehab assignments in Illinois. The role focuses on evaluating and treating patients with functional and cognitive impairments within SNF and long-term care settings.

You will develop and implement individualized care plans, collaborate with a multidisciplinary team, and ensure documentation meets regulatory standards while maintaining physician orders.

Qualifications

• OT degree from an AOTA-accredited program.
• NBCOT certification as an OTR (if state requires).
• Current Occupational Therapist license in the practicing state.
• Strong communication and organizational skills.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure physician orders are obtained and up to date.
