Job summary

Cornerstone Rehab is seeking a PRN Certified Occupational Therapy Assistant to deliver care to our patients and collaborate with a diverse team of clinical professionals at various centers across the region.

You will provide occupational therapy to adult and geriatric patients, prepare progress notes and discharge summaries, and establish a plan of care tailored to each patient’s needs. Travel to nearby centers as needed is required.

Qualifications

• Valid State Occupational Therapy Assistant License and Credentials
• SNF and/or Acute care experience

Responsibilities

• Provide occupational therapy to adult and geriatric patients
• Prepare documentation, progress notes, and discharge summaries
• Establish a plan of care in accordance with patients' needs and capabilities
• Travel to nearby centers as needed
