Job summary

Houston Transitional Care in Houston, TX seeks a Staff Occupational Therapist to evaluate and treat patients, develop plans, and lead therapy teams. You will train staff, document care, and coordinate with physicians and families to ensure optimal rehabilitation outcomes.

The role requires licensure, strong communication, and the ability to interpret policies. You will participate in care conferences and maintain accurate medical records while supporting a multidisciplinary team.

Qualifications

• Bachelor's degree in Occupational Therapy (preferred Masters/Doctorate).
• Proficient using Casamba and Point Click Care software.
• Licensed Occupational Therapist in the state and maintain licensure.

Responsibilities

• Evaluate patients within 24 hours of referral; document reasons for delays if any.
• Develop treatment plans and obtain physician approval.
• Treat patients per the physician plan and document progress.
• Supervise Occupational Therapy Assistants and student staff.
• Communicate progress with supervisor and health team.
• Participate in care conferences and rehabilitation meetings.
