Qualifications

• Excellent communication and interpersonal skills with a patient-centered approach.

Responsabilités

• Patient assessment: evaluate physical, emotional, and cognitive abilities using observations and standardized tools.
• Develop and implement personalized treatment plans to meet client goals and improve function.
• Maintain accurate documentation of evaluations, progress and treatment plans in accordance with policies.
• Advocate for client needs and coordinate with care teams and community resources.
