Job summary

HealthPro Heritage is seeking a PRN Occupational Therapist to join our team at National Church Residences Bristol Village and Chillicothe. You will assess patients, develop personalized treatment plans, educate families, and maintain thorough documentation.

Must hold a degree in Occupational Therapy and valid state licensure, or be eligible for licensure. Roles include direct patient care, advocacy, and collaboration within a multidisciplinary team in nursing facilities, hospitals, and home care

Qualifications

• Education: Degree in Occupational Therapy from an accredited institution.
• Licensure: Valid state licensure as an Occupational Therapist, or license eligible.
• Strong clinical assessment and diagnostic skills; proficient in developing and implementing effective treatment plans.
• Excellent communication and interpersonal skills with a patient-centered approach.
• Physical requirements: Ability to lift and assist patients; stand for extended periods.

Responsibilities

• Patient Assessment: Evaluate clients' physical, emotional, and cognitive abilities using observations and standardized tools.
• Treatment and Education: Create personalized treatment plans to meet clients' goals and educate on daily activities and adaptive equipment.
• Documentation: Maintain accurate records of evaluations, progress, and treatment plans.
• Advocacy and Support: Advocate for clients' needs and support access to community resources and accommodations.
