Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our dynamic SNF team. You will evaluate and treat patients in an individualized plan of care and collaborate with facility staff to deliver excellent patient outcomes.

The role includes developing plans of care, setting measurable goals, educating patients and families, and supervising Occupational Therapy Assistants. Strong documentation and communication are essential to success.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
