A leading home health agency seeks an Occupational Therapist to provide coordinated care to clients of all ages. The role involves assessing patient needs, developing care plans, and providing therapeutic interventions to enhance their well-being and functional independence. Ideal candidates will have a degree in Occupational Therapy and at least two years of experience in a clinical setting, preferably in home health care.

Occupational Therapist
General Summary: Provide coordinated care to clients of all age groups. Plans, implements, and evaluates client care plans to restore or maintain patient well-being. Evaluates occupational roles and performance of clients with physical, psycho-social and/or cognitive impairments. Provides intervention to deficits and elicits responses.
Patient Population: Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with
patients in all populations.
Essential Functions:
1. Perform clinical assessments; identifies patient needs and appropriateness of client.
2. Document, prepare/revises and completes a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing
basis.
3. Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA).
4. Assess and facilitate through direct therapeutic intervention client’s ROM, strength, coordination needs for and use of adaptive equipment and
environmental modifications, functional mobility and safety issues as related to client’s ADL performance needs.
5. Provides effective communication to clients, their families, team members, and other health care professional, and regular participation in Case
Conference/Team Meetings. Facilitates the use of these skills by other team members to achieve desirable outcomes.
6. Evaluate client occupational performance in the areas of self-care, work, leisure, and rest. Assess sensory-motor neuromuscular, psycho-social, and
cognitive-perceptual components of occupational behavior.
7. Monitors assigned cases to ensure compliance with requirements of third-party payers.
8. Completes appropriate documentation in a timely manner to assure compliance with company policy.
9. Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency.
10.Meet mandatory continuing education requirements of the Agency/licensing board.

On-going employment as an OT always requires maintenance of a valid OT license in the state which the therapist practices in and a valid CPR certificate. It is the responsibility of the employee to renew these before they expire in order to continue employment.
