Cleveland Clinic Health System is seeking a PRN Occupational Therapist to join the Rehab OP team at Green Health and Wellness in Uniontown. You will deliver therapy focused on restoring independence for patients with neurological and other conditions, including hand therapy and potential CHT pathways.

Qualifications

• Bachelor’s degree in Occupational Therapy.
• Graduate of an AOTA-accredited OT program.
• Current Ohio OT license.
• BLS certification.

Responsibilities

• Evaluate and treat patients with physical, emotional, cognitive and/or psychosocial deficits affecting independence.
• Assess patient ability to safely perform independent living tasks.
• Design the plan of care, including type and frequency of treatment.
• Implement the treatment plan using appropriate modalities.
• Document subjective and objective patient progress findings.
• Monitor and modify the plan of care as needed.
• Seek consultation as necessary.
• Educate patient and family about deficits and care options.
• Assist with discharge planning from therapy services.
• Oversee OT assistants and therapy technicians per licensing laws.
