Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and create a plan of care.
• Use standardized assessments and evidence-based practices to guide interventions.
• Provide rehabilitative, skilled, and medically necessary treatment per regulations.
• Administer procedures as part of the rehab plan (ADL retraining, exercises, modalities, devices).
• Monitor progress and adjust treatments in the clinical record.
• Evaluate outcomes and refer as needed based on progress.
• Provide counseling to patients, families, and caregivers about the disorders.
