Benedictine Living La Crosse is seeking an Occupational Therapist to evaluate residents, develop and implement therapy plans, and contribute to discharge planning. You will lead the care plan development and coordinate with the care team to meet resident goals.

Responsibilities include ongoing assessment, establishing outcomes, and educating residents, families, and caregivers about the plan of care. This is a casual/PRN role within a long-term care setting, located in La Crosse, WI.

Qualifications

• Current state licensure as an Occupational Therapist, in good standing.
• Bachelor’s degree.

Responsibilities

• Evaluates residents and develops plan of care based on needs.
• Establishes functional goals, outcomes, and discharge plan.
• Plans for discharge and anticipates long/short term needs.
• Educates health care team, residents, family, and caregivers on care plans.
