Job summary

Continuum Therapy Partners is seeking an Occupational Therapist for PRN daytime shifts at locations in Mill Creek and Galion, Ohio. You will evaluate, plan, and implement individualized treatment programs to improve patients’ functional independence while documenting progress and updating plans based on outcomes.

The role requires an OT license (NBCOT) and a master's or doctoral degree, with 1 year of experience preferred.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state OT License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
