Continuum Therapy Partners is seeking an Occupational Therapist to join a therapist-led model across a national footprint. This PRN role in Zumbrota, MN focuses on evaluating patients, developing individualized plans, and enhancing functional independence for diverse populations.

The successful candidate will hold an OT degree, current MN license, NBCOT certification, and prefer 1 year of experience. No remote work; on-site care at the Zumbrota location with supportive clinical leadership.

Competenze

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred

Mansioni

• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
