Current state licensure or eligibility for state licensure as a Physical Therapist required.
Certified BLS or completion in the first 90 days of employment required.

Responsibilities

Perform comprehensive evaluations and develop individualized treatment plans for patients in medical-surgical, ICU, telemetry and specialty units.
Participate in interdisciplinary rounds and collaborate on discharge planning for safe patient transitions.
Document timely and accurately in the electronic medical record per hospital and regulatory standards.
Provide patient/family education on mobility, safe transfers and home exercise programs as appropriate.
Contribute to quality improvement and evidence-based practice initiatives within the team.
