Responsibilities

• Perform comprehensive evaluations and develop individualized treatment plans for patients in medical-surgical, ICU, telemetry and specialty units.
• Participate in interdisciplinary rounds and collaborate on discharge planning for safe patient transitions.
• Document timely and accurately in the electronic medical record per hospital and regulatory standards.
• Provide patient/family education on mobility, safe transfers and home exercise programs as appropriate.
• Contribute to quality improvement and evidence-based practice initiatives within the team.
