Job summary

AllStar Therapies is seeking a compassionate PRN Occupational Therapist (OT) to deliver skilled nursing therapy services in a SNF setting. You will evaluate residents, create individualized treatment plans, and document progress according to physician orders.

You will collaborate with physicians, nursing staff, rehabilitation team, residents and families to support independence and quality of life. PRN flexibility and a supportive team environment are highlighted.

Qualifications

• Strong communication and organizational skills are required.
• Graduate of an accredited Occupational Therapy program.
• CPR certification is preferred but not required.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide occupational therapy services per physician orders.
• Help residents improve activities of daily living.
• Document evaluations, treatments, progress notes, and discharge summaries accurately and timely.
• Collaborate with physicians, nursing staff, rehabilitation team, residents, and families.
• Participate in interdisciplinary care planning.
• Ensure compliance with all state, federal, and company regulations.

Responsibilities

• Evaluate residents and develop individualized treatment plans

• Provide occupational therapy services in accordance with physician orders

• Help residents improve their ability to perform activities of daily living

• Document evaluations, treatments, progress notes, and discharge summaries accurately and timely

• Collaborate with physicians, nursing staff, rehabilitation team, residents, and families

• Participate in interdisciplinary care planning

• Ensure compliance with all state, federal, and company regulations

Qualifications

• Current Occupational Therapist (OT) license in the state of practice

• Graduate of an accredited Occupational Therapy program

• Previous experience in skilled nursing or rehabilitation is preferred but not required

• Strong communication and organizational skills

• Ability to work independently while collaborating with an interdisciplinary team

• CPR certification preferred
