Job summary

Premise Health in Wisconsin is seeking a Nurse Practitioner for PRN coverage in our DeForest and Madison clinics. You will provide primary care and occupational health services to employees and dependents, with potential temporary full-time coverage in January–February 2027.

Responsibilities include physicals, order/interpret tests, care planning, and patient education, in coordination with the TPA and client.

Qualifications

• Active RN license and NP license/certification in the practice state(s).
• Current national certification (ANCC or AANP).
• Master’s in Nursing; transcripts if not clearly MS-Nursing.
• CPR/BLS and ACLS as required by contract scope.

Responsibilities

• Perform physicals, preventive care, and fitness-for-duty exams.
• Provide primary and occupational health care per guidelines.
• Develop care plans, document progress, and ensure continuity.
• Order/interpret diagnostics; collaborate with clinicians for care.
• Prescribe treatments, refer to specialists as needed.
