Job summary

CenterWell Senior Primary Care in Fort Worth, TX, is seeking a Nurse Practitioner to deliver comprehensive primary care to seniors in an outpatient setting. You will work with an interdisciplinary team, document in Athena EMR, and focus on direct patient care.

This role offers a collaborative, patient-centered environment with a Monday–Friday schedule, on-site clinics, and a pay range reflecting market levels for a full-time NP in Texas.

Qualifications

• Master's degree in nursing with board certification.
• Active, unrestricted NP license in the state of practice.
• Minimum two years' Nurse Practitioner experience.
• EMR experience and adapting to new technologies.
• Participation in TB screening program.
• Commitment to improving patient experience and outcomes.
• Preferred: value-based care or Medicare Advantage with geriatrics.

Your Role & Responsibilities

Required Qualifications

• Participation in Tuberculosis (TB) screening program.

Preferred Qualifications

• Experience in Value-Based care, Primary Care or Medicare Advantage settings working with Geriatric patients.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license or Medicaid and Medicare numbers.
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Work Environment

• Outpatient clinical setting.
• Our clinics are open Monday-Friday 8am-5pm.

Scheduled Weekly Hours

• Availability for 3 days a week preferred
• Home Clinic: CenterWell Seminary, 4200 South Freeway, Suite 1990, Fort Worth, TX 76115
  • Other locations on as-needed basis: CenterWell Camp Bowie, 9108 Camp Bowie West, Fort Worth, TX 76116, and CenterWell Arlington, 2562 W. Pioneer Pkwy, Pantego, TX 76013

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$149,800 - $206,100 per year
