Job summary

Qualifications

Responsibilities

• Conduct comprehensive assessments of patients across physiological, psychological, developmental, sociocultural and spiritual factors.
• Develop and implement individualized care plans; reassess and revise as needed.
• Administer medications and treatments per physician orders and protocols; monitor for adverse reactions.
• Educate patients and families about conditions, treatments, and self-care to promote understanding.
• Collaborate with medical staff to coordinate patient care and ensure effective communication.
• Document patient information, care activities, interventions and outcomes accurately.
• Evaluate care plan effectiveness and adjust to improve patient health outcomes.
• Ensure infection control and safety compliance.
• Provide emotional support and therapeutic communication to patients and families.
• Supervise and delegate care within the healthcare team; maintain quality and efficiency.
• Engage in performance improvement activities for patient care and operations.
• Respond to codes promptly and provide interventions to stabilize patients.
