Job summary

Christian Living Communities in Pueblo West is seeking a compassionate PRN Personal Care Partner II (QMAP) to support residents with daily care and medication assistance at Cappella Pueblo West. This role emphasizes safety, dignity, and person-centered care in a community setting.

You will collaborate with the care team to meet residents' needs and help maintain a warm, welcoming environment. Qualified candidates will have QMAP certification, English proficiency, and preferably one year of elder

Qualifications

• Medication certification required (QMAP-A/L, Med Tech, Med Aide, Nurse Delegation).
• Must read, write and speak English.
• One year elder care experience preferred.
• Strong self-management and organization skills.
• Ability to handle confidential information.

Responsibilities

• Assist residents with personal care such as bathing, dressing, meals, laundry and daily routines.
• Support medication administration by following procedures and documenting accurately.

Our PRN team members assist the Community by covering shifts that become open either from PTO, or call offs.

At Cappella Pueblo West, we believe in a Culture of Citizenship — where each person brings unique gifts, perspectives, and value to our community. We are seeking a caring and dependable Assisted Living Personal Care Partner II (QMAP) to support residents with daily care and medication assistance.

In this role, you will help residents with personal care needs such as bathing, dressing, meals, laundry, and daily routines. You will also support medication administration by following established procedures, documenting accurately, and helping ensure residents receive the right medication at the right time.

The ideal candidate is compassionate, observant, and committed to resident safety and well-being. You will communicate changes in resident needs, support families as appropriate, and work closely with team members to provide respectful, person-centered care.

Qualifications

Basic Qualifications & Experience

• Per the state of operation requirements, Medication certification; QMAP-A/L, Med Tech, Med Aide, Nurse Delegation
• Must be able to read, write and speak the English language.
• High school or Equivalent (GED)
• One (1) year experience in some level of elder care preferred
• Proficient computer skills preferred
• Strong self-management and organization skills
• Strong multi-tasking abilities with the ability to handle competing deadlines, flexible and adaptable (Open to change).
• Ability to effectively handle difficult and sensitive issues and maintain confidentiality

Working Conditions / Physical Requirements

• Moderate physical activity required by handling objects up to 50 pounds occasionally and/or up to 20 pounds frequently.
• Constant standing, walking, and grasping. Frequent stooping and reaching above shoulder heights, occasional sitting, crawling, kneeling, and squatting.
• May have exposure to blood borne pathogens, body excretions and communicable diseases possible.
• Possible exposure to household chemicals.
• Sits, stands, bends, and moves intermittently during working hours.
• May be subject to frequent interruptions.
• Interacts with clients, family members and visitors frequently.
