PRN Medical Social Worker MSW (Education)

Job Title/Position: Medical Social Worker (MSW)

Reports To: Director of Social Services

Job Description Summary

The medical social worker, contracted or employed through the organization, is responsible for implementing standards of care for medical social work services and ensuring adherence to all conditions in the Service or Employment Agreement.

Position Qualifications
  • Has a Master of Social Work (MSW) degree from an accredited school of social work and maintains a license from the State Board of Social Work for each state where work is performed.
  • Has at least one year of social work experience in a healthcare setting (hospice experience preferred).
  • Understands hospice philosophy and issues related to death and dying. Experience in hospice care is preferred.
  • Demonstrates good verbal and written communication skills, along with organization skills.
  • Possesses a valid driver's license with an insured automobile in good working condition.
Essential Job Functions/Responsibilities
  1. Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment, communicating findings to the interdisciplinary team.
  2. Provide assessments in patients' residences and assist with safety plans when necessary.
  3. Conduct social evaluations and plan interventions based on findings.
  4. Counsel patients and families/caregivers regarding stress and coping difficulties.
  5. Maintain clinical records for all referred patients.
  6. Provide information and referral services to address practical and environmental needs.
  7. Advocate for patients and families regarding health benefits, providers, and resources.
  8. Serve as liaison between patients, families, and community agencies.
  9. Maintain collaborative relationships within the organization to support patient care.
  10. Develop contracts with public and private agencies as resources.
  11. Participate in care planning and interdisciplinary meetings, including QAPI activities.
  12. Assist physicians and team members in understanding social and emotional factors, even if the patient declines social work services.
  13. Participate in discharge planning and assist with legal and end-of-life arrangements as needed.
  14. Provide guidance and supervision to social work assistants as assigned.
  15. Perform other duties as delegated by the Hospice Director/Administrator.
Working Conditions

Community and home environment, exposure to infectious diseases, in and out of automobile. Employees may be involved in potential exposure to bloodborne pathogens and will be offered Hepatitis B vaccination at no expense.

Physical Requirements
  • Sufficient visual/hearing ability to comprehend written/verbal communication.
  • Ability to perform physical tasks, including heavy lifting, bending, and standing.
  • Adaptability to patients' needs and status.
Continuing Education Requirements

Meet state licensure continuing education hours and participate in mandatory and additional educational activities to enhance skills. Attendance at agency-mandated educational programs is required.
