Role Overview

The Social Worker assists patients and families in coping with severe or long-term illnesses. Responsibilities include assessing, diagnosing, and treating mental and social conditions, counseling individuals and families, and maintaining case records.

Responsibilities

• Assess psychosocial status and emotional factors related to the client’s health and treatment response.
• Provide direct social casework services, including counseling and intervention planning.
• Advise on community resources, eligibility, and regulations.
• Participate in developing and updating the client’s care plan.
• Monitor and report client responses to treatment.
• Maintain communication with care teams and document client information accurately.
• Attend care conferences, staff meetings, and agency committees.

Qualifications

• Thorough knowledge of social work principles and methods.
• Ability to work effectively with diverse populations and complex situations.
• Excellent communication skills, both oral and written.
• Master’s degree in Social Work is required.
