Job summary

Guardian Angel Home Care is seeking an experienced Medical Social Worker (MSW) to join our home health team on a PRN/per diem basis. This role offers flexibility, supplemental income, and strong compensation without a full-time commitment.

The position requires an MSW and a full, active social work license. Travel to patients' homes within the service area is expected, with visits and documentation completed accurately and timely.

Qualifications

• MSW required and active unrestricted license
• Limited licenses not accepted
• Experience in home health, hospice, hospital, rehabilitation, or community-based settings preferred

Responsibilities

• Complete psychosocial assessments
• Evaluate patient and family needs
• Provide counseling and emotional support
• Connect patients with community resources and support programs
• Assist with financial, transportation, housing, and long-term care resources
• Support patients and families with adjustment to illness or disability
• Participate in interdisciplinary care planning
• Coordinate with nurses, therapists, physicians, and other clinical staff
• Document visits and interventions accurately and timely
• Travel to patients’ homes throughout the assigned service area
